Sub:     Individual monthly performance review (ACs, DCs, ADCs)

       In order to ensure an objective and transparent monthly performance review of every officer (AC, DC, ADC), it is required to furnish related information on attached standard format.

2. Officer will fill in the attached Performa and submit the same on 6th of each month relating to previous month. Performance for July-Sep 2013 is to be submitted by 28.10.2013.

3.         If any officer think that his/her name of job/performance cannot be filled to tabulated in the given performa, he/she may furnish the same in a precise and quantified manner. This should reflect achievement/performance during the period under review.
